Attach File icon opens the Add Note dialog (All submit and update forms)

All submit and update forms in RLC share a common toolbar. When you click that Attach File icon on this toolbar, the Add Note dialog opens. We have verified this problem on Submit and Update for:

When submitting issues via mail submit which have a .txt attachment what happens is that the attachment is imbedded into the item and not attached to the item as it should be.
