Multi-selection field is shown as listbox instead of checkboxes

If you try to create a form action to trigger when a multi-selection field (configured for checkbox style) loses or gains focus, it will only work if the user is checking or unchecking the first checkbox. All other checkboxes will not initiate a callback.

Checkboxes are displayed for each line as well as the "Check All" and "Uncheck All" elements below the report. The Check All and Uncheck All links do not function.

This happens with multi - selection , multi -relational, multi -groups and multi -user fields that are setup to render in Checkbox style. When a field is setup for checkboxes, each available option is displayed with a checkbox and a corresponding label. If the user clicks the checkboxes, everything is fine.

When building a form (either custom or based on 'quick' form), if the form contains binary or multi - selection fields displayed as radio buttons or checkboxes , the Preview in Composer displays the proper arrangement, but the form in the end-user's Web page is not correct.
