CMS-XML Attachments removed from request when it's updated simultaneously by multiple users via Web Client.
If a request is updated by two or more users simultaneously, via Web Client, there is a chance that attachments are removed from request when last user saves request updates. To prevent this user must open the General tab and verify that attachments are still listed there. User can then save the update from within General or any other tab.

Unfortunately the only option you have is to check out problem description and then check it back in using a different revision number (or use the UI command), then you can delete the original revision.
