When the user entered the actual server name (IP address) to connect, she received the above database connection error. When she entered the logical name for the server under 'server name' she received the error 'pvcs tkn has caused an invalid page fault.'
Win2000, Tracker 7.0, Oracle 8i. When trying to log in from the Tracker Inet Database Connection screen to make a project available to the web the following error is returned: "ORA-12638 Credential retrieval failed"

For debug this situation I enabled SDP trace logs and found an event trigger called old connection name. I think that's why action driven deployment is failed .
