2. Create a sub workflow right under the parent (Not at the same level ) 3. In the sub workflow create a couple overides on the Submit transition like make description required and put a custom form on the Submit. 4. Check everything in and deploy
"You can override help text provided for fields on custom forms by adding text to the Description box for the field control. This setting is located on the General tab of the field control editor for a form.
