By default this is the IUSR account. On a Windows 2008 system the IIS_IUSRS group has permission to this file but the IUSR account isn't a member of this group by default. It's up to you how you do this you can either add the IUSR account to the IIS_IUSRS group or you can just explicitly add the IUSR account to the web.config file and give it full permissions.

Giving the "Everyone" group full control over the queue will always work and is the default on a standard Windows 2003 Server install. For Windows 2008 this isn't the default and needs to be done manually. Your companies group policy may override this. If you notice that messages are appearing in the MSMQ queue but don't ever leave the queue, it is likely the correct permissions are not set. Start by giving Everyone full control over the queue.
