When trying to create or delete a work item you get Backlog Name (xxx) has been modified since last retrieval and item won't create or delete.
You may start to get this error after changing the Date and Time settings or Time Zone settings on either the Agile server or the Database server if the two boxes are different. In order to fix this run the following sql statement against the Agile database and log back into Agile and try the create or delete again. update PES_Object set obj_LastChange = GETDATE()

How to Reset the User Settings
For example, all of the views remember your last setting. So, if your datasheet (or report) always gives you an error but works fine for others, the problem could be with the dates you had selected last time the report was run. The dates you entered last time are "remembered" by the system as part of your user settings.

Failed login attempt with LDAP authentication causes two tries for each login.
If you have ldap authentication setup and attempt to login as one of the ldap users with a bad password then agile makes two attempts to connect and causes the users account to be locked out in Active Directory before the domain group policies limit. In other words if your group policy says accounts should be locked out after 3 failed attempts you will actually get locked out if you put in the wrong password in Agile just twice because it tries 2 times for each, so four tries with just two failed attempts.

Why can't I login to my On Demand product?

How Do I Add Personal Vacation Time?
Your personal calendar can be updated from the Workspace module in the My Calendar view. The personal calendar for other users may be changed in the Resources module in the Resource Pool view. However, only a user with Administrator rights may access these views.

Troubleshooting Microsoft Message Queue (MSMQ) Problems or Notification sending problems.
Setup a very simple notification definition that will send an email to your normal login user (not peadmin/aodadmin). Make sure the notification is very simple, such as one that fires every time a project (or story) is updated . Also, make sure it is very generic without any filters or restrictions.

IE hangs and never responds, or IE freezes for a long time and then the Mariner login screen appears
Users are actively working in Mariner when IE will suddenly stop responding. Sometimes the browser will hang and the Internet Explorer process will need to be stopped via Task Manager on the client. Other times, after a freeze time of three or four minutes, the browser will refresh itself and take the user back to the Mariner login screen.
